Giant Frontoethmoidal Mucocele: Case Report And Review Of Literature.

Abstract

In this case we present a 39years old lady, with a history of head injury at five years of age. She presented to our facility with a twenty-eight years history of a large recurrent frontal swelling associated with deteriorating vision in the right eye. Physical examination revealed a large cystic mass on the right forehead with associated proptosis and mechanical ptosis. Vision in the right eye was 6/24. Notably, she had two unsuccessful attempts at endoscopic drainage fifteen years ago. CT scan images revealed a large cystic mass within an expanded right frontal air sinus and associated erosion of the anterior wall of the sinus. The right globe was displaced infero-laterally with obvious proptosis noted. The patient underwent craniotomy for excision of the mass and cranialization. An old fracture was noted in the posterior wall of the sinus. Thereafter she underwent orbital reconstruction by the maxillofacial and ophthalmology teams. This case illustrates a rare complication of skull fractures involving the frontal sinuses. The authors have illustrated the intraoperative details of this case and also highlight the role of a multidisciplinary team in the management of the lesion.
